Certified or not, to become a member of the Buckeye SCUBA Club simply provide us with:

1. A completed OSU Recreational Sports "Code of Conduct and Emergency Contact" at this website.

2. A completed "Dive History Quesionnaire."

3. A completed "Standard Safe Diving Practices."

4. Club dues ($20).

5. If being certified, a copy of the "Statement of Understanding."

6. A photocopy of your SCUBA certification card (ONLY if already certified).

All this paperwork is a pain, but due to the innate dangers of SCUBA diving- ALL PAPERWORK MUST BE PROPERLY COMPLETED before anyone can join the club.

 

We will be more than happy to accept new memberships at any of our weekly meetings.
